UC Hastings requires all enrolled students to have health insurance coverage. UC Hastings has partnered with the University of California to offer students access to the University of California Student Health Insurance Plan for graduate students (UC SHIP). If you have other major medical health insurance comparable to UC SHIP and don't wish to have the advantages of dual coverage, then you can submit an online Health Insurance Waiver form to discontinue your enrollment in UC SHIP and receive a credit for the health insurance fee. Students who successfully waive UC SHIP must have proof of coverage by another medical health insurance plan that meets UC Hastings' minimum criteria.
All students are automatically enrolled in and billed for UC SHIP every Fall. Students who have successfully waived out of UC SHIP in the past, must re-submit new waivers every Fall.
What is the UC Graduate Student Health Insurance Plan (UC SHIP)?
UC SHIP is a comprehensive major medical insurance plan, providing medical, counseling, prescription, vision and dental services. The year-round plan, which uses the Anthem Blue Cross PPO network, is tailored to meet the health and wellness needs of graduate students. Remember, all students can use Student Health Services, whether they have UC SHIP or not.
Considering waiving out of Student Health Insurance?
If you already have health insurance coverage, and that insurance plan meets UC Hastings' minimum criteria, you may request to waive out of UC SHIP by submitting a waiver form online. Before you complete the waiver form, first verify your current insurance plan provides primary care and emergency care in the San Francisco Bay Area. Some insurance plans require that you visit their own network providers to receive care, and in some cases, such networks may not be available within the San Francisco Bay Area.
If you have verified that network coverage is available within the San Francisco Bay Area, please complete the waiver criteria worksheet prior to proceeding. If you do not have the answers to the questions listed on the worksheet, you should contact your insurance company to collect all the information prior to completing the online waiver form.
Waivers must be submitted each academic year.

What are the requirements for waiving out of UC SHIP?
In order to waive enrollment in UC SHIP, you must have a medical insurance plan that meets ALL of the following UNIVERSITY OF CALIFORNIA minimum health care criteria:
UC SHIP Waiver Criteria, effective Fall 2015
1) Be a Medi-Cal, Medicare or Tricare/military insurance policy or a Covered California plan
2) Be an employer-sponsored group health plan or individual plan that meets the following criteria:
i. Preventive health care services, including an annual physical exam, preventative immunizations and
laboratory/diagnostic tests to help determine your state of health
ii. Chronic disease management for such conditions as asthma, diabetes or other chronic medical conditions
iii. Hospital stays for medical and surgical care
iv. Hospital stays for mental health and alcohol/drug abuse conditions, covered the same as any other medical
v. Doctor office visits for medical, mental health, and alcohol/drug abuse conditions
vi. Emergency room services
vii. Diagnostic services including laboratory tests
viii. Medications prescribed by a doctor (including contraceptives)
ix. Pre-natal and maternity care, with no pre-existing condition limitation
1) have no pre-existing condition exclusion; if the plan has a pre-existing condition waiting period, that period has expired
2) have no per-injury or per-illness maximum benefit limits
3) cover medical services for injury from participation in all types of recreational activities or amateur sports
4) not be a health care reimbursement arrangement with the student’s home country or another party.
5) have policy written in standard English with benefits expressed in U.S. dollars
6) have a claims payment office with an address in the United States
7) pay at least $50,000 annually for medical evacuation
8) pay at least $25,000 for repatriation of remains
REMEMBER! You can submit the waiver form one time ONLY. After the waiver from is submitted, you will receive a “waiver Accepted” or “waiver Denied” page based on the answers provided on the waiver from. You will NOT be able to access the waiver system again to make any corrections, so it is important that you have all the correct information before submitting the waiver form.
